A parents biggest fear is “what happens to our disabled child when they reach adulthood an/or we are no longer able to care for them?” Heartstrings Foundation was created specifically for this reason! Every individual, regardless of abilities or disabilities, should be able to move out in their own community, engage with with their peers and enjoy their own independence!
Types of Community Residential Providers
Community Residential Providers are contracted statewide with the Developmental Disabilities Administration (DDCS) to provide residential services to adult clients. Services include:
- Supported Living
- State Operated Living Alternatives
- Group Training Homes or Group Homes
- Alternative Living
- Companion Homes
What is Supported Living?
Supported Living provides personalized, in-home care for adults with developmental disabilities or mental health issues, enabling them to live independently in their own homes rather than institutions. Services include 24-hour support, assistance with daily tasks, medication management, and community integration, often provided by contracted, specialized agencies
Types of Services provided by Heartstrings Foundation Supported Living Agency:
- Support and training in obtaining residence
- Assistance with rental agreements
- Support and assistance applying for SNAP, SSI, other services
- Obtaining funds through DDCS for transitioning into new residence
- Daily activities
- Transportation
- Financial management/assisting/ Payee
- Nursing Services
- Direct Support Professionals
- CNA’s/HCA’s if needed
- Nurse Delegation
- Assistance in finding/keeping housemates
- Personalized care by people who have been on both sides of services
